- Christopher L. Lykens, 38, Youngstown, was westbound on Lewis Road in Franklin Township at 4 p.m. Oct. 22 and attempted to turn into a private drive and went left of center and was struck by Jason O. Cunningham, 32, Reef Road NE, Mechanicstown, who was also westbound on Lewis Road. No citations were issued.

-William M. Persohn, 64, Mick Road, Wellsville, was westbound on state Route 39 in Washington Township at 1:55 p.m. Sunday and Nicole Renee Vest, 30, Rose Drive, Lisbon, was northbound on Steubenville-Pike and failed to yield turning left and struck Persohn. She was cited for right of way.

- Donald K. Foster, 50, state Route 7, Lisbon, and Jack L. Salsberry, 60, Echo Dell Road, Rogers, were southbound on state Route 7 in Madison Township at 11:50 a.m. Sunday and Foster stopped waiting to turn left into a private drive and Salsberry failed to stop and struck his vehicle. Salsberry was cited for assured clear distance ahead.

- Jason E. Carter, 36, Rochester Road, East Rochester, was southbound on Georgetown-Damascus Road in Butler Township at 8:14 p.m. Oct. 22 and drove off the right side and hit a street sign.

- Ludovica L. Diaiandomenico, 75, Aliquippa, Pa., and Janetta Starr Pippert, 65, Enon Valley, Pa., were westbound on state Route 14 in Unity Township at 10:30 a.m. Thursday and Pippert attempted to turn left onto Howell Road and was struck by Diaiandomenico. Diaiandomenico was cited for assured clear distance ahead.
